Overview

Triamcinolone/Nystatin/Neomycin/Gramicidin is a combination topical medication containing a corticosteroid (triamcinolone acetonide), an antifungal (nystatin), and two antibiotics (neomycin sulfate and gramicidin). It is used to treat inflammatory skin conditions complicated by bacterial and/or fungal infections.

Mechanism of Action

Triamcinolone acetonide is a synthetic corticosteroid that suppresses inflammation by inhibiting inflammatory mediators. Nystatin binds to ergosterol in fungal cell membranes, causing membrane permeability and cell death. Neomycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis. Gramicidin is a polypeptide antibiotic that disrupts bacterial cell membrane integrity.

Indications

  • Inflammatory dermatoses (eczema, dermatitis) with secondary bacterial or fungal infections
  • Corticosteroid-responsive dermatoses complicated by infection

Dosage

Apply a thin layer to affected area 2-4 times daily. Duration should be limited to avoid systemic absorption and adverse effects.

Contraindications

  • Hypersensitivity to any component
  • Viral skin infections (herpes simplex, varicella)
  • Tuberculous or fungal skin lesions not responsive to antifungals
  • Perioral dermatitis
  • Acne vulgaris

  • Hypersensitivity reactions
  • Skin atrophy with prolonged use
  • Pigmentation changes
  • Hirsutism
  • Acneiform eruptions
