Overview
Tretinoin is a topical retinoid used primarily for the mitigation of fine facial wrinkles as an adjunct to comprehensive skin care and sunlight avoidance programs. It works by modulating epidermal differentiation and proliferation, though it does not eliminate wrinkles or reverse photoaging. Treatment requires medical supervision and strict sun protection measures.
Mechanism of Action
Tretinoin is a retinoid that binds to retinoic acid receptors, modulating gene expression to normalize keratinocyte differentiation, reduce hyperkeratinization, and promote epidermal cell turnover. It also inhibits comedogenesis and has anti-inflammatory properties.
Indications
- Adjunctive treatment for mitigation of fine facial wrinkles in patients using comprehensive skin care and sunlight avoidance programs
Common Doses
- 0.02% cream
- 0.05% cream
- 0.1% cream
- 0.025% gel
Dosage
Apply a pea-sized amount to the face once daily in the evening after cleansing and drying skin. Wait 20-30 minutes after washing before application. Avoid eyes, ears, nostrils, and mouth. Effects may take up to 6 months to appear.
Contraindications
- History of hypersensitivity to any component
- Pregnancy or attempting pregnancy
- Sunburned skin
- Eczema or other chronic facial skin conditions
- Inherent sensitivity to sunlight
- Concurrent use of photosensitizing drugs
Side Effects
- Peeling, dry skin, burning, stinging
- Erythema, pruritus
- Severe skin irritation requiring temporary discontinuation
- Hypo- or hyperpigmentation
- Local reactions at application site
Interactions
- Photosensitizing drugs (thiazides, tetracyclines, fluoroquinolones, phenothiazines, sulfonamides) increase phototoxicity risk
- Concomitant topical medications may increase irritation
- Abrasive products, drying cosmetics, and alcohol-based products may exacerbate irritation
Counseling Points
- Use strict sun protection including SPF 15+ sunscreen and protective clothing
- Apply only pea-sized amount once daily in evening
- Avoid sun exposure and tanning beds
- Expect initial irritation that may improve with continued use
- Do not use on sunburned or irritated skin
- Results may take several months to appear
- Discontinue if severe irritation occurs
