Etamsylate

Brand Names: Dicynone

Drug Class: Hemostatic agent, Capillary stabilizer

Overview

Etamsylate (Dicynone) is a hemostatic agent used to control bleeding by promoting platelet adhesion and capillary wall stability. It's commonly prescribed for various bleeding disorders and to reduce bleeding during surgical procedures.

Mechanism of Action

Etamsylate enhances platelet adhesion by increasing platelet aggregation and restoring capillary resistance. It acts on the first phase of hemostasis by promoting the formation of platelet plugs at sites of vascular injury without affecting coagulation factors.

Indications

  • Treatment and prevention of capillary bleeding
  • Menorrhagia (excessive menstrual bleeding)
  • Postpartum hemorrhage
  • Bleeding in surgery (ENT, ophthalmology, urology)
  • Prevention of bleeding in patients with thrombocytopenia

Dosage

Adults: 500 mg 2-4 times daily. Children: 10-15 mg/kg/day in divided doses. For surgical bleeding: 500 mg IV/IM before surgery and 500 mg every 6 hours postoperatively. Duration depends on clinical response.

Contraindications

  • Hypersensitivity to etamsylate or any component
  • Acute porphyria
  • Severe renal impairment
  • History of thrombosis or thromboembolic disorders

Side Effects

  • Headache
  • Nausea
  • Diarrhea
  • Rash
  • Dizziness
  • Hypotension with rapid IV administration
  • Fever
  • Injection site reactions

Interactions

  • May enhance effects of other hemostatic agents
  • No significant interactions with anticoagulants reported
  • Potential interaction with NSAIDs (theoretical increased bleeding risk)
