Overview

Estradiol is a bioidentical estrogen hormone used to treat symptoms of vulvar and vaginal atrophy associated with menopause. It works by replenishing estrogen levels in vaginal tissues to improve symptoms like dryness, itching, and painful intercourse. Treatment should be limited to the shortest duration needed and regularly reevaluated.

Mechanism of Action

Estradiol binds to estrogen receptors in target tissues, including the vaginal epithelium, leading to cellular proliferation and restoration of normal vaginal mucosa. It increases vaginal blood flow, secretions, and elasticity while reducing pH.

Indications

  • Treatment of moderate to severe symptoms of vulvar and vaginal atrophy due to menopause

Common Doses

  • 0.01% vaginal cream

Dosage

Initial dose: 2-4 g vaginally daily for 1-2 weeks, then reduced by half for similar period. Maintenance: 1 g vaginally 1-3 times weekly after mucosal restoration. Use shortest duration possible with periodic reevaluation.

Contraindications

  • Undiagnosed abnormal genital bleeding
  • Known, suspected, or history of breast cancer
  • Known or suspected estrogen-dependent neoplasia
  • Active DVT, PE or history of these conditions
  • Active arterial thromboembolic disease (stroke, MI) or history
  • Anaphylactic reaction or angioedema to estradiol
  • Liver dysfunction or disease
  • Protein C, protein S, or antithrombin deficiency or thrombophilic disorders
  • Known or suspected pregnancy

  • Breast tenderness/pain
  • Headache
  • Nausea
  • Abdominal pain
  • Fluid retention
  • Mood changes
  • Increased risk of thromboembolic events with systemic absorption

Interactions

  • May alter coagulation parameters (prothrombin time, platelet function)
  • Increases thyroid-binding globulin - may require thyroid hormone dose adjustment
  • Alters corticosteroid and sex hormone binding globulin levels
  • Affects lipid profiles (increases HDL, decreases LDL, increases triglycerides)
  • May impair glucose tolerance

Counseling Points

  • Use only as prescribed for shortest duration needed
  • Apply using measured applicator as directed
  • Report any unusual vaginal bleeding immediately
  • Regular breast exams and mammograms recommended
  • Monitor for signs of blood clots (leg pain, chest pain, shortness of breath)
  • Avoid use during pregnancy or breastfeeding
  • Store at room temperature
